- Nominator: The individual declaring this person to be a curmudgeon
- While there is a raging debate as to whether a person can self-identify as a curmudgeon, we are agnostic on this issue
- Date: Defaults to date certificate is printed, BUT a different date can be selected - such as birthday, Christmas, etc.
